Raffle Prize Claim Disclaimer
The claimant acknowledges and agrees that all raffle prizes are awarded “as is” without warranty or guarantee, express or implied, except for any manufacturer warranty that may apply. The claimant is solely responsible for any applicable federal, state, or local taxes, fees, licenses, permits, registrations, background checks, transfer requirements, or other costs associated with claiming, receiving, possessing, or using the prize.

Eligibility to receive a prize is subject to all applicable laws, regulations, raffle rules, and eligibility requirements. If the claimant is legally prohibited or otherwise ineligible to receive a prize, the prize may be forfeited or handled in accordance with the official raffle rules and applicable law.

Prize Availability and Substitution: All prizes are subject to availability. If an advertised or awarded prize becomes unavailable, is discontinued, cannot be produced or supplied, or otherwise cannot reasonably be provided, the raffle organizer reserves the right to substitute the prize with another item of equal or comparable value. Any substitution will be determined by the raffle organizer based on availability and other relevant circumstances. Except for such substitutions, prizes may not be exchanged, transferred, or redeemed for cash unless expressly authorized by the raffle organizer.

Authenticity and Verification: All prize claims are subject to verification by the raffle organizer prior to the release or transfer of any prize. The claimant may be required to provide the original winning raffle ticket, claim form, valid government-issued identification, or other documentation reasonably necessary to establish the claimant’s identity, eligibility, and entitlement to the prize. The raffle organizer reserves the right to verify the authenticity and validity of any raffle ticket, entry, claim form, or supporting documentation against its official records. Any ticket, entry, or documentation that is altered, duplicated, counterfeit, illegible, tampered with, improperly obtained, or otherwise cannot be reasonably authenticated may be deemed invalid. No prize will be released until the raffle organizer is reasonably satisfied that the claim is valid and that all applicable eligibility and verification requirements have been met.

By accepting the prize, the claimant acknowledges that the raffle organizer, its owners, officers, employees, agents, sponsors, and affiliates are not responsible for any loss, damage, injury, claim, or liability arising from the acceptance, possession, use, misuse, or transfer of the prize, except to the extent such liability cannot lawfully be waived.

The claimant certifies that all information provided on this claim form is true and accurate and acknowledges that providing false, misleading, or incomplete information, or submitting fraudulent or altered documentation, may result in disqualification or forfeiture of the prize.
